This is a highly advanced professional position that is a member of the Mobile Response Team providing crisis services, screenings, mental health and substance abuse assessments, evaluation intake, treatment planning and documentation addressing the specific treatment needs of clients, as well as consultation services to Center and other agency staff. This position works closely with schools, law enforcement and families in the community.

Care Coordinator – New Mobile Response Teams

Position Summary:
This is an independent professional position performing assessments, service brokerage, and advocacy services for persons up to the age of 25 that experiences a behavioral health crisis that meets criteria for care coordination as a response through the Mobile Response Team. Care Coordinators works closely as a part of the Mobile Response Team staff assisting when there is a crisis call, responding when needed as an on-call advocate. The Care Coordinator identify and engage eligible individuals in services, with the goal of reducing admissions to the CSU and assuring follow-up appointment within 72-hours. The Care Coordinator links clients with community, natural, and professional support systems to meet socioeconomic and physical and behavioral health care needs, and provides psychoeducational interventions to assist in recovery and wellness. Care coordination is intended to be short term and intensive. The work is performed with a high degree of independence; however, work is reviewed through conferences, supervision and required reports.  Work is performed in the community, schools, hospitals, client homes, CSU and Detox, and Meridian offices and clinics.

Qualifications
